I believe quests were changed so that only the person sent to look for it can find the item now.

Yay I was hunting for something in Shame and was there for 2 other repspawns (I think) before I found it. Now to the reason for my post:
When you take an item quest from the AG is the item added during the new respawn? Are the items randomly generated when you kill a MOB? I would think they are randomly generated cause wouldnt that be easier for the server? Im not that .... technically adept so I am probily wrong

That sounds possible and logical. But you probably want to ask on the Tech forum for a definitive answer. I think Drocket scans that forum more often.
